Health Care Plan Cancellation Survey Terms and Conditions
- Companion Care (Services) Limited (company registered number 04141142) of Epsom Avenue, Stanley Green Trading Estate, Handforth, Cheshire, SK9 3RN is the promoter of a scheme to gather insights into the cancellations of “Health Care Plans” (the “Promoter” and the “Scheme” respectively).
- The following rules apply.
- By submitting information, you will be deemed to have accepted and be bound by these terms and conditions and you are confirming the following:
- The information provided as part of my Submission is true and correct.
- I am aged 18 or over.
- I shall be liable for any and all losses, and all claims by third parties, suffered by the Promoter resulting from my breach of any of these terms and conditions.
- Any personal information received by the Promoter in the course of the Scheme will be used in accordance with the Promoter’s privacy policy.
- These terms are governed by the laws of England and Wales and in the event of a dispute the parties submit to the exclusive jurisdiction of the Courts of England and Wales.
